About the Role

Our South West Community Team within Older Person's Health Service provides assessment and rehabilitation of elderly clients within the community. We're an interdisciplinary team working with clients in the community with a goal of maintaining patient independence. Our service is forward thinking, utilising InterRAI to effectively assess the needs of our clients. We also have a restorative model of care and our work is focused on listening to our clients and helping to support and enable people to remain independent in the community for as long as possible.

We have a fixed term part time opportunity based at The Princess Margaret Hospital working 36hrs per week (0.9FTE) for an experienced Social Worker with a current Annual Practicing Certificate to join our South West Community Service Team!

We operate a Monday ' Friday service. The days and hours of this role are to be negotiated with the successful candidate, and to meet the needs of the service.

In this role, you will be responsible for:

  • Assessment, monitoring and support of clients within the service in accordance with ANZASW Code of Ethics and Standards of Practice
  • Providing appropriate resource and education to clients and their family/ whānau
  • Participating in service development and quality assurance initiatives / processes

About You

Bring your passion for improving the health outcomes of our elderly community

Your skills will include:

  • Excellent interpersonal skills with the ability to build and nurture relationships with clients and their family/whānau
  • The ability to work collaboratively within an interdisciplinary team
  • Relevant experience working with elderly is preferable
  • Experience within a health setting is advantageous
  • Current Annual Practicing Certificate issued under the Social Workers' Registration Act (2003) is essential
