About the Service

The Infant2Teen Health Assessment is based on a free holistic health assessment for children (0-17 years), within the context of their whānau and community setting. It is completed by a Clinical Nurse Specialist (CNS) who is employed by Health New Zealand, Te Whatu Ora - Waitaha. The CNS is required to have appropriate postgraduate qualifications in Child and Youth Health, alongside comprehensive understanding of family violence dynamics, child protection issues and New Zealand legislation.

The intention of the health assessment is to acknowledge the child's and their whānau perspective of their health and life journey to date, alongside accessing professional input from as many services as possible. This includes:

  • Health services (GP, Te Whatu Ora, Well Child Provider, B4 School Provider etc.)
  • Community and/or statutory services
  • Education (Early Childhood, Primary and High Schools, associated learning and behaviour services etc.)
  • Other services as determined necessary in consultation with the whānau
  • As required, completion of child and parental health screens
  • A limited physical assessment by the CNS for the child/young person with medical follow up by the GP as required.

About the Role

In the form of a written report, the CNS utilises collated information, updated clinical knowledge and researched evidence to make recommendations for the child and family to consider. A copy of the Infant2Teen Health Assessment Report is given to:

  • The whānau; an abridged report and a full report. This provides them with the option to share either copy with another service in the future
  • The child's GP and Te Whatu Ora Waitaha
  • The Referrer
  • Other services in consultation with the whānau

The Infant2Teen Health Assessment Service is coordinated by a CNS on a full-time basis with 20 hours of administrative support. This non-acute service is fully funded by Health New Zealand Te Whatu Ora - Waitaha. All work completed on behalf of the whānau is underpinned by the Principles of the Treaty of Waitangi.

At times, other resource demands are placed on the Child and Family Safety Service (C&FSS) and may impact on the delivery of the Infant2Teen Health Assessment Service. This situation is regularly reviewed by the CNS and the C&FSS Manager, in consultation with The Chief of Child Health as required. For this reason, the number of health assessments to be completed in any given month may vary. The waitlist requires regular reviewing to ensure that consideration can be given for likely timeframe of appointments.

Rather, it is about working alongside the whānau and their referrer during the assessments, maintaining short term case management, completing updated written reports and recommendations so that they are relevant at the time of discharge from the service. However, not all children and young people will need a full health assessment, there may be health needs identified that can be addressed by enhancing whānau and referrers health literacy, and/or navigating pathways within the community or hospital system.

About You

Essential:

  • Registered with the Nursing Council of New Zealand with a current APC.
  • Have or actively working towards a relevant Post-graduate diploma.
  • Demonstrate advanced nursing skills comparable to senior nurse or expert PDRP level.
  • Demonstrate skills in nursing leadership and teaching/mentoring/coaching skills.
  • Computer literate and has a drivers licence.
  • Proven expert clinical assessment skills and care and protection knowledge.
  • Proven ability to support nursing and other staff at all levels of practice.
  • Demonstrated ability to develop appropriate standards/policies and quality initiatives.
  • Has ability to work autonomously and act as part of a team, supporting other staff at all levels of practice.

Desirable:

  • Obtained or working towards relevant Clinical Masters (NACNS 2009).
  • Demonstrates ability to develop nursing standards and quality initiatives
  • Demonstrate involvement in research that has changed nursing practice.
  • Senior Nurse competent PDRP portfolio.
  • Knowledge of relevant legislation and regulations.
  • Smoking Quit Card Provider
  • Knowledge and experience with contraception.
